Step 1: The Consult
Call or text (970)823-4826 and leave a message containing your name, phone number, and we will return your call as soon as possible. (This is an outgoing line only and responses are only made to voicemails.)
OR
Email info@JGardnerAndAssociates.com (or Josh@JGardnerAndAssociates.com to coordinate a phone call) and let us know your name, contact information, how many conditions you want addressed in your nexus letter(s), and if all of those issues have been directly addressed in your medical record.
Our providers do not speak to clients via phone. We understand that your time is valuable, but so is ours and we’ve found that if people, just want to chat about their claim, have not read our website for prices, the process, and other FAQ then they are most likely not actually interested in our service. Spending time answering repetitive questions or receiving documents we haven’t even asked for takes time away from our paying clients.

DO NOT INCLUDE SENSITIVE MEDICAL INFORMATION IN A VOICEMAIL OR  TEXT!

PLEASE BE ADVISED THIS E-MAIL ADDRESS IS NOT ENCRYPTED END-TO-END AND SHOULD NOT BE USED TO PROVIDE ANY PERSONAL PHI (Protected health information) OR ELECTRONIC MEDICAL RECORDS.
PLEASE ENSURE ALL PHI IS SUBMITTED TO THE PASSWORD PROTECTED E-MAIL SUPPLIED BY YOUR PROVIDER TO ENSURE COMPLETE SECURITY AND CONFIDENTIALITY.

Step 2: The Contract and Invoice
We will send the link and password to the contract page on this website and secure invoice to your email. We do not receive any of your payment information.

Step 3: Medical Records Review
After you have been approved through our consult process we will send instructions on how to send us your medical records.
We value your privacy and follow HIPAA guidelines to protect your information. We are excited to have a HIPAA compliant email to allow our clients to send their records electronically! (Emailed records should only be sent to our Proton address Records@JGardnerAndAssociates.com AFTER YOU HAVE RECEIVED A PASSWORD PROTECTED EMAIL FROM THIS ADDRESS) We no longer accept records via mail.
As the VA representatives suggest it is wise to highlight or organize your medical records per issue for review. You know your record better than anyone. (A fee may be added for unorganized records)
Please do not send us records before your contract is signed and invoice is paid. We review medical records… please do not send selfies, texts, social media posts, etc.

Step 4: The Nexus Letter
After your records have been reviewed if we are confident that we can provide you with a quality Nexus Letter that positively states that your condition is linked to your service the provider will complete your letter. Yes, we do write “at least as likely as not” and/or “more likely than not” in every letter.
If we are unable to medically link your condition with your service you will be notified via email and we will let you know what we need to see to continue with your letter. If you are unable to provide us with the evidence we need then that is the end of our association until such evidence can be provided. We will not entertain discussions regarding your opinion that we should believe differently, but if you feel we missed something in your records or you will be able to provide us with said documentation then we will continue when it is received. You will not be charged for the Nexus Letter, but the fee for the consult/records review is non refundable.

Step 5: Receiving Your Letter
Once your email is verified we will email you a password protected .pdf copy of your letter. Upon payment we will send you the password. If you find any errors they will be corrected, but the medical opinion will not be changed.
